• कुटुंबाचे पुणे महानगरपालिका हद्दीत किमान ३ वर्षे वास्तव्य असल्याचा दाखला (रेशनिंग कार्डची साक्षांकित प्रत आवश्यक) तसेच ३ वर्षे वास्तव्य असलेल्या राहण्याचा पुरावा म्हणून गत ३ वर्षाचा मनपा टॅक्स पावती किंवा लाईट बिल किंवा टेलिफोन बिल (लॅण्डलाईन) किंवा झोपडी फोटो पास / झोपडी सेवा शुल्क पावती / भाडे करारनामा इ. जोडावे.
• सर्व वयोगटातील अपंग (नि:समर्थ) व्यक्ति (वयाचा पुरावा म्हणून जन्मदाखला / ससून जनरल हॉस्पिटलमधील वयाचा दाखला, शाळा सोडल्याचा दाखला, मतदान यादीतील नाव अथवा मतदान ओळखपत्र यापैकी एक आवश्यक)
• दिनांक ०१/०५/२००१ नंतर जन्माला आलेल्या व हयात अपत्यांमुळे कुटुंबाच्या अपत्यांची संख्या दोनपेक्षा जास्त झाल्यास योजनेचा लाभ मिळणार नाही.
• मा. राज्य शासनाने ठरवून दिलेल्या टक्केवारी नुसार ४०% किंवा त्यापेक्षा जास्त अपंगत्व (नि:समर्थ) असल्याचा दाखला तसेच शासन वेळोवेळी जे बदल सुचवतील त्या प्रमाणात नियम व अटीत बदल करण्यात येईल.
• अर्जदाराच्या आधार कार्डची झेरॉक्स प्रत सादर करावी.
• पुणे महानगरपालिका अपंग स्मार्ट कार्ड असल्यास झेरॉक्स प्रत जोडणे आवश्यक.
• अटी व नियम यांत बदल करण्याचा / अर्ज नाकारण्याचा अधिकार मा.महापालिका आयुक्त अथवा ते प्राधिकृत करतील त्या अधिकाऱ्यास राहील.
• स्कॅन डॉक्युमेंट्स संलग्न करणे आवश्यक आहे.
• अर्ज सादर करताना मूळ कागदपत्र (original documents) सोबत आणावेत.

Required Documents for Free Bus Pass Scheme

1. Certificate of residency of the family for at least 3 years (attested copy of rationing card required) as well as proof of residency for 3 years as per the Municipal Corporation Tax receipt or light bill or telephone bill (landline) or hut photo pass / hut service fee receipt / Lease agreement etc.
2. Persons with disabilities of all ages (Birth certificate as proof of age / Age certificate from Sassoon General Hospital, School leaving certificate, Name in the voter list or Voter ID card required)
3. If the number of children in the family exceeds two due to surviving offspring born after 01/05/2001, the benefit of the scheme will not be availed.
4. Proof of disability (disability) of 40% or more as per the percentage fixed by the state government as well as changes in terms and conditions as suggested by the government from time to time.
5. Xerox copy of Aadhar card of the applicant should be submitted.
6. If you have Pune Municipal Corporation Disabled Smart Card, Xerox copy is required.
7. The power to change the terms and conditions / reject the application rests with the Municipal Commissioner or the officer authorizing it.
8. Scan documents must be attached.
9. Original documents should be brought along while submitting the application.
