अंध, अपंग, विकलांग व्यक्तिंना कृत्रिम अवयव घेणेसाठी योजना-(DWS)

Financial Assistance for Aids and Appliances for Disabled Persons Scheme Provide necessary tools to the needy disabled according to their personal needs. There are also plans to provide artificial limbs, three-wheeled bicycles to suit their age or needs. In terms of physical rehabilitation, calipers, artificial limbs for the disabled, three-wheeled bicycles, hearing aids for the deaf and spectacles for the blind, sticks and a set of tape recorders and ten blank cassettes for blind students to complete their education. The Applicants monthly income under this scheme is up to Rs. 1,500 / – will be given 100% subsidy for purchase of organs and tools. The Monthly income of Applicants between Rs.1,501 / – to Rs.2,000 / –  will be given 50% subsidy. The scheme will be applicable to adults once in 3 to 5 years and to children below 15 years of age every year. The limit of financial assistance under this scheme is at least Rs. 25 / – and a maximum of Rs. 3,000 / -. In the case of artificial limbs, 15% of the cost of implantation will be paid to the beneficiaries.

अंध, अपंग, विकलांग व्यक्तिंना कृत्रिम अवयव घेणेसाठी या योजनेंतर्गत मासिक उत्पन्न रु. 1,500/- पर्यंत असणाऱ्या अर्जदारास अवयव व साधनांच्या खरेदीसाठी 100% तर मासिक उत्पन्न रु. 1,501/- ते 2,000/- पर्यंत असणाऱ्या अर्जदारास 50% अथसहाय्य देण्यात येईल. ही योजना प्रौढ व्यक्तींसाठी 3 ते 5 वर्षातून एकदा आणि 15 वर्षाखालील मुलांना दरवर्षी लागू असेल. या योजनेंतर्गत अर्थसहाय्याची मर्यादा किमान रु. 25/- व कमाल रु. 3,000/- अशी राहील. कृत्रिम अवयवांच्याबाबतीत अवयव बसविण्यासाठी आकारण्यात येणाऱ्या खर्चाच्या 15% रक्कम लाभार्थ्यांस देण्यात येईल.

अंध, अपंग, विकलांग व्यक्तिंना कृत्रिम अवयव घेणेसाठी या योजने मध्ये शारिरीक पुनर्वसनाच्या दृष्टीने अस्थिविकलांग व्यक्तींकरीता कॅलिपर्स कृत्रिम अवयव, तीन चाकी सायकल, कर्णबधिरांकरीता श्रवणयंत्र व अंध व्यक्तींना चष्मे, काठया तसेच इयत्ता दहावी नंतरचे शिक्षण पूर्ण करण्यासाठी अंध विद्यार्थ्यांना टेप रेकॉर्डर व दहा कोऱ्या कॅसेट्सचे संच दिले जातात.

  1. श्रवणयंत्रासाठी अर्जदाराची वयोमर्यादा वय वर्षे 55 पर्यंतच आहे. अर्जासोबत नाक-कान-घसा तज्ञ डॉक्टरच्या सही शिक्क्यानिशी श्रवणऱ्हासाचा आलेख जोडणे आवश्यक आहे. शाळेत शिकणाऱ्या नोकरी किंवा व्यवसाय करणाऱ्या व्यक्तीसच श्रवणयंत्र दिले जाते. या कार्यालयाकडील मान्यताप्राप्त कंपनीकडूनच श्रवणयंत्र घेणे बंधनकारक असते. श्रवणयंत्रासाठी वैयक्तीक अर्ज केलेल्या अर्जदाराच्या चौकशीनंतर चौकशी अहवाल आल्यावरच श्रवणयंत्र दिले जाते.
  2. तीन चाकी सायकलसाठी अर्जदाराचे वय किमान 18 वर्षे पूर्ण असावे. अर्जदार एकाच पायाने अधू नसावा. दोन्ही पायांनी अपंग व्यक्तीसच तीन चाकी सायकल दिली जाते. वैद्यकीय दाखला/अपंगत्वाचे प्रमाणपत्र स्वयंस्पष्ट असावे. तसे नसल्यास अपंगत्व स्पष्ट होईल असे अर्जदाराचे छायाचित्र सोबत जोडणे आवश्यक आहे.

How to apply Financial Assistance for Aids and Appliances Scheme for Disabled Persons

  1. कृत्रिम अवयव, कॅलिपर्स इ. साठीच्या अर्जासोबत अपंगत्वाचा स्पष्ट उल्लेख असणारा वैद्यकीय दाखला जोडावा. त्या अपंगत्वानुसार लागणाऱ्या कृत्रिम अवयव/साधनांचा अर्जात स्पष्ट उल्लेख असावा. अर्जदाराला यापूर्वी या कार्यालयाकडून कोणत्या स्वरुपाची मदत मिळाली हे नमूद करावे.

Eligibility for Financial Assistance for Aids and Appliances Scheme for Disabled Persons

  1. The Applicants monthly income under this scheme is up to Rs. 1,500 / – will be given 100% subsidy for purchase of organs and tools.

  5. The age limit of the applicant for hearing aids is 55 years.
  6. The application must be accompanied by a signature stamp from a nose-ear-throat specialist doctor. Hearing aids are given only to those who have a job or business in school. It is mandatory to get hearing aids from an approved company from this office. Hearing aids are provided only after the inquiry report is received from the applicant who has personally applied for the hearing aid.
  7. Applicants must be at least 18 years of age for a three wheeler. The applicant should not be left alone. Only a disabled person with two legs is given a three wheeler.
  8. Medical certificate / disability certificate should be self explanatory. Failure to do so should be accompanied by a photograph of the applicant indicating disability.
  9. Artificial limbs, calipers etc. A medical certificate with a clear mention of disability should be attached with the application for.
  10. The application should clearly mention the artificial limbs / devices required for that disability. The applicant should state the form of assistance received from this office earlier.

Required Documents for DWS under PMC

  1. रेशनिंग कार्डची स्वसाक्षांकित प्रत अपत्य पडताळणीकरीता आवश्यक.
  2. कुटुंबाचे पुणे महानगरपालिका हद्दीत किमान ३ वर्षे वास्तव्य असावे तसेच ३ वर्षे वास्तव्य असल्याचा पुरावा म्हणून गत ३ वर्षाचा मनपा टॅक्स पावती किंवा लाईट बिल किंवा टेलिफोन बिल (लॅण्डलाईन) किंवा झोपडी फोटो पास/झोपडी सेवा शुल्क पावती/भाडे करारनामा इ. जोडावे.
  3. वयाचा पुरावा म्हणून जन्मदाखला/ससून जनरल हॉस्पिटलमधील वयाचा दाखला, शाळा सोडल्याचा दाखला, मतदान यादीतील नांव अथवा मतदान ओळखपत्र यापैकी एक आवश्यक.
  4. दिनांक १/५/२००१ नंतर जन्माला आलेल्या व हयात अपत्यांमुळे कुटुंबाच्या अपत्यांची संख्या दोन पेक्षा जास्त झाल्यास योजनेचा लाभ मिळणार नाही.
  5. कुटुंबाचे सर्व मार्गानी मिळालेले वार्षिक उत्पन्न रू. १ लाखाच्या आत असावे. (अर्जदार झोपडपट्टीत राहत असल्यास शेजार समुहगटाचा उत्पन्नाचा दाखल्याबाबतचा ठराव जोडावा आणि गावठाणात राहत असल्यास मा. तहसिलदार यांचेकडील उत्पन्नाचा दाखला जोडावा.).
  6. अर्जदाराचे आधार कार्ड व बॅंक पास बुक झेरॉक्‍स प्रत जोडणे आवश्यक आहे.
  7. अनुदानातून जे साहित्य (कॅलीपर, तीन चाकी सायकल, बुट, हिअरींग एडस्) घेणार त्याचे मुळ कोटेशन जोडावे
  8. कृत्रिम अवयव घेणेबाबत तज्ञ डॉक्टरांचे (अस्थिरोग तज्ञ, कान-नाक- घसा तज्ञ, नेत्र तज्ञ इ. )शिफारस पत्र जोडणे आवश्यक आहे.

  • विहीत नमुन्यातील अर्ज सादर करावा.
  • लाभार्थ्यांच्या पालकाचे उत्पन्न दरमहा रु 1500/- पेक्षा कमी असावे. रु 1501/- ते 2000/- पर्यंत उत्पन्न असणाऱ्या व्यक्तींना सदर साधनाची अर्धी रक्कम भरावी लागेल.
  • अर्जदाराचे अपंग किमान 40 टक्के अथवा त्यापेक्षा जास्त असावे.
6. दिल्या जाणाऱ्या लाभाचे स्वरूप या योजनेत अस्थिव्यंग अपंगांसाठी कृत्रिम साधने (कॅलिपर्स, बूट, पाठीचे जॅकेट) कृत्रिम अवयव, तीन चाकी सायकली, मूकबधिरांना श्रवण यंत्रे, अंधांना चष्मे,पांढरी काठी इत्यादी रु. 3000/- पर्यंतचे साहित्य देण्याची योजना आहे.
